After our doctor approves and registers you, the state will send you an email linking back to their website, where you must log-in and complete your patient profile. Following their instructions, you must submit an electronic copy of government-issued photo identification (driver’s license, passport, or military ID) with your proof of address, a clear recent photo, and the last four numbers of their Social Security number.
