Where to Apply to get a Florida Medical MMJ Card?

How Do I Apply - Application Submission Process

Doctor Evaluation

  • As shown above, Florida law requires you to upload a copy of the necessary proof of residency. All patients or caregivers must upload a copy of a valid Florida driver's license or Florida identity card. A smartphone pic works well.
  • Notably, the Registry connects to the Division of Highway Safety and Motor Vehicle’s State ID system. They match your contact information with your state ID or driver's license and automatically add your ID photo to your file.
  • Your name and address on the document(s) provided must match the name and address submitted in the application. To put it another way, uploading a picture of your license matches precisely what they already have on file. It makes no sense to throw them any curveballs.
  • As soon as your photo and proof of residency have been uploaded, sign your online application. Be sure to type your name precisely as they appear on your online application in the appropriate boxes.

 

Submit and Payment

  • Once you click "Submit My Card Application," a button will appear: "Click Here to Pay Online." Click that button to access the Bill2Pay system to supply your payment information. Credit cards and eChecks are accepted. A $2.75 convenience fee applies. If the bank declines your payment, the state charges a $15 service fee to your account.
  • Renewal applications should only be submitted before the expiration date, up to 45 days before your card expires. If you wait until after the expiration, it will take another 10 - 14 days for the state to process a new one. You will not be able to purchase medicine during this time.

What Happens Next? - OMMU Application Approval Process

Online payments take five business days to clear. Once your payment clears, the OMMU begins its review. This review process takes another five business days. After the State completes their assessment, they will send you another email. In the meantime. You use this email as a temporary card at the medical MMJ treatment center while waiting for your CBD card. The State mails the official one within another five business days. If they reject any part of an application for missing or invalid documentation, they delay the processing time. Patients can purchase medical cannabis approximately 10-14 days after they visit KindHealth for their card. Many dispensaries need an in-person visit first, before arranging a home delivery.

Seasonal Resident Proof of Residency

Seasonal residents may not have a valid Florida driver license or Florida ID card. Unlike full-time Florida residents, they must upload a copy of two of the following documents:

  • A deed, mortgage, monthly mortgage statement, residential rental or lease agreement, or a mortgage payment booklet.
  • Provide one proof of a Florida residential address. The proof can be from the seasonal resident’s parent, step-parent, legal guardian, or another person with whom the seasonal resident lives. You must include a statement from the person with whom the seasonal resident lives, stating that you live with him or her.
  • A utility hook-up or work order dated within 60 days before registration in the medical use registry.
  • A utility bill needs to be less than two months old.
  • Mail from a financial institution, including checking, savings, or investment account statements, needs to be less than two months old.
  • Correspondence from a federal, state, county, or municipal government agency, needs to be less than two months old.

 

Minor patients must submit a certified copy of a birth certificate or a current record of registration from a Florida K-12 school. Also, you must include a copy of the minor’s parent or legal guardian’s valid Florida driver’s license or ID.
