What if I Need to Change my Card Information?
Our office can make limited changes to your medical cannabis card. If changes are needed, call us first to discuss them. The Office of Medical Marijuana Use needs to make many other changes. Requests to replace a lost or stolen card will require the cardholder to submit a Change, Replacement, or Surrender Request Form, along with a copy of his or her Florida driver’s license or identification card and a $15 replacement fee.
To update your Florida medical marijuana card, follow these active steps:
Fill Out the Form:
- Use the Change, Replacement, or Surrender Request Form to update your details.
- Attach a copy of your Florida driver’s license or ID or a legal document like a marriage certificate or divorce decree for a name change.
- If you want to change your address, provide a copy of your Florida driver’s license, ID, or other proof of residency.
Mail the Form and Fee:
- Send the completed form with a $15.00 fee (if needed) to the Office of Medical Marijuana Use.
For Caregivers:
- New caregivers must complete the Medical Marijuana Use Registry Identification Card Caregiver Application.
- Legal representatives must show proof of representation when changing or adding a caregiver.
If Your Card is Lost, Stolen, or Damaged:
- State the reason for the replacement on the form and include the necessary documents.
For Minors:
- A parent or guardian must sign all forms and provide a copy of their Florida driver’s license or ID.
